I sent by snail mail to the folks at Chase bank.
The text of the letter follows.
Chase Card Services, EFS Dear Sirs: I was told by the folks at the Arizona Department of Economic Services who handle their unemployment to complain to you and say that the debit card they issued to me was either lost or stolen. I got the above address by calling the number 866-802-7011 which was given to me by the folks at the Arizona DES. Back on October 8, 2010 I was laid off from my job and the following week I applied for unemployment. As with most government agencies Arizona DES is an overstaffed and over paid bureaucracy and it is a pain in the butt dealing with them. After about 4 or 5 weeks of dealing with the idiots at Arizona DES told me that I did qualify for unemployment benefits and they told me they mailed me a debit card. I didn't even know I qualified for the debit card, and for that matter I thought I was appealing their rejection of my claim. I was not even aware that they had sent me a debit card. But since I just found out they mailed me a debit card and that I have not received the card yet, I am now writing you to notify you that the card was lost and probably stolen. I have no idea what the card number is because the folks at DES never told me that. The folks at DES told me not to give you my social security number so I didn’t include that.
